APPLICATION FOR EMPLOYMENT
Thank you for your interest in working with Living at Home Health Services, LLC. We are an established LLC that helps people who need a little assistance at home. We do not provide skilled home health care, but partner with other local agencies (i.e., Chester River Home Health & Hospice, Health Department personal care programs) to provide continuity of care and maximize our local resources so the patient can safely stay at home.
Please complete the application below. Be prepared to provide us with copies of:
- Valid Drivers License
- Social Security Card
- CPR/First Aide Certification (ONLINE CERTIFICATIONS ARE NOT ACCEPTED)
- CNA/ GNA Medication Certification (NOT REQUIRED)
- TB Test Results (Can be done at your local Health Department, your doctor’s office or Health Center)
- Hepatitis B Vaccination (your immunization record is acceptable)
You are required to be fingerprinted. Download the fingerprint authorization form and take it to either the Queen Anne’s County Sheriff’s Office in Centreville, MD (Monday–Friday 8:30 am – 3:30 pm).
